2nd flex plate failure

About 3 months ago the flex plate in my sons 96 trooper broke. I got a new flexplate (6 hole). I replaced the Torque Converter(3 Studs) Put new line up dowels in.(There was one missing) Put in factory bolts and replaced the bushing in the end of the crank. I bought the Trooper used and the flex plate broke within a week.Now the flex plate is broke again. There has to be something that is not lined up. The engine was replace at sometime in the rigs life. Help, I need some ideas
